and Linda Clark, dated June 19, 1996, recorded in Deed Book 107,

been given to secure a Note of even date in the original principal
amount of Two Hundred Forty Thousand and 00/100 Dollars ($240,000.00)
with interest thereon as provided for therein, said Security
Deed having been last sold, assigned and transferred to First

The indebtedness secured by said Security Deed has been and is
hereby declared due because of default under the terms of said
Security Deed and Note, including but not limited to the nonpayment

Announcement of
Intent to Procure
The Northeast Georgia MHMRSA Regional Board intends to make available
to interested providers a Request for Information (RFI) document
which seeks mental health and substance abuse services provision
to children and adolescents through the Medicaid Community Mental
Health Center Program.
The purpose of this Request for Information (RFI) is to provide
interested groups and organizations with sufficient information
to enable them to prepare and submit responses for consideration
by the Regional MHMRSA Boards. It provides general information
for the development and submission of responses along with the
description of services solicited.
The Northeast Georgia Regional MHMRSA Board is issuing this RFI
in order to identify an additional children/adolescent provider
to offer Medicaid Community Mental Health Program services as
certified Medicaid providers. These providers will be selected
by Regional MHMRSA Boards and will be recommended to the Department
of Community Health's Division of Medical Assistance as eligible
providers to whom that Division should offer a Medicaid Provider
Agreement.
Through this announcement, the Regional Board wishes to create
a mailing list for the distribution of the RFI document. Because
the Departments and Boards are interested in expanding the provider
pool for public behavioral health services, offerors under this
RFI may not include Community Service Boards, since community
Service Boards are already Medicaid-funded Community Mental Health
Program providers.
If your agency has an interest in receiving a copy of this RFI,
you must submit a written request (including address, phone number,
and an identified contact) to this office.
